How i process the correction of gender and middle initial on my birth certificate

I was 18 years old when I noticed that my gender on my birth certificate shows that I am a male. During that time, if you’re going to fix it, it will cost you a lot. Around 40k++ and it will also take so much of your time since you need to attend to some hearings.

Around 40k++ and it will also take so much of your time since you need to attend to some hearings.Knowing that I can’t afford to fix it at that time, i just let it pass,  intending not to fix it at all.
Until the Republic Act 10172 passed. Where correction of gender is less complicated.
I was very thankful for it.
It was June 28, 2018 when I completed and submit all the requirements. (Took me almost a month because of conflict of schedule)
Here’s the details on how I did it.
Disclaimer: All the details is based on my personal experience. My status at the time is single and unemployed so you may have a different requirements depending on your status.
First thing first!
You need to go to your LCR (Local Civil Registry) where you were registered.(In my case, I am registered in Marikina so I went to our Local Municipal Hall)
It’s better if you bring a PSA copy of the birthcertificate so that they can check all the details that needs to be corrected.
(In my case, I don’t have it with me and just told them I needed a correction of gender and ask for a requirements.)
So these are the requirements:
(I handwritten it,notice the red and black mark. I’ll explain it as we go along)
  • Birth certificate – should be PSA copy
  • Police clearance – (This is only valid for 4mos so I suggest you get this last) P150.00. In order for you to get this, you should have a cedula (I paid P5.50 since I’m unemployed. Get it at your municipal city) and baranggay clearance (P10.00 at your baranggay office)
  • NBI Clearance- P130 but I paid P155.44. You should log in FIRST at https://clearance.nbi.gov.ph. Answer the online form, write down the generated code, make a payment(I made an online payment, the fee will change depending on the mode of payment you chose) and go to any NBI office and show them the code. They will take a photo of you, fingerprint and signature. If you don’t have a hit, you’ll get it right away.
  • Form 137 – this is your elementary school records from grade 1-6. You need to give them the year you graduated for them to easily find your record. It is free in my school.
  • Affidavit of non employment – P200.00. You can just go to private law office for this or to your municipal’s office if you want it in public. In my case, I found out it’s much cheaper in private.
  • Affidavit of entry to be corrected – P200.00. Same description as above.
  • Baptismal Certificate – P50.00 the church where you were baptized doesn’t necessarily mean that they have your record. In my case, it was forwarded to another church but same baranggay. They will ask you of your Christening date and year for them to easily find your record.
  • Medical Records showing entry to be corrected – this is a 2 copies of paper given by the LCR onset of your inquiry. It was divided into 2 parts, the first part needs to be signed by the doctor who will examine you and the 2nd part needs to be signed by the LCR.
  • Medical Certificate – P150.00 this will be given once you have been examined by an OBgyne from a government owned hospital (important, since LCR will not accept it if it’s done by a private obgyne) The obgyne may or may not require for a vaginal ultrasound. In my case, I opt to have my ultrasound (P600.00 by appointment) first at AMANG Rodriguez Hospital before I was examined by a Doctor. Medical Certification will be given in less than a week.
  • Two valid ID’s
As i was about to submit my requirements, i was told to include the correction of my middle name since it will cost me the same and that it will become my future problem if in case I’ll get a passport. On my birth certificate, it only shows my middle initial V. wherein it should be Valles.
I was asked to provide additional requirements for this. You can go back to the photo above and notice the black checks.
  • Mother’s birth certificate
  • Father’s birth certificate
  • Parent’s marriage certificate
  • Siblings birth certificate – at least 2 siblings.
I photocopied all the requirement by 2 and went back to LCR.
I was given 3 sets of paper which shows “Annex A” together with all my requirements and asked me to go to legal office which is on the second floor of our Municipal Hall.
As the legal officer is filling up the “Annex A” form through the typewriter, he asks me a few questions while enumerating my requirements
  • Affidavit of discrepancy – P300.00. When the legal officer ask me about it, I told him I don’t have one so he asked his colleague to make one for me. Later that day, I realized I paid for it because the Affidavit of entry to be corrected that I have only shows the correction of my Gender, excluding the correction of my middle name.
I was asked again to go back to LCR with all the papers still with me.
I photocopied the Correction of discrepancy first before submitting it to LCR.
LCR then told me to pay P3,000.00 to the casheir and asked me to have the receipt photocopied.
After that, she asked me to pay P2,100.00 for the printing news and delivery (no receipt for this) Then she attached the copy of the receipt to my requirements. And she gave me the original receipt attached to one copy of “Annex A” form with a contact number and asked me to call back after a month
